Yesterday I had a pt that was ETOH, he was goofing around with his gf on bikes (surprise no helmets) and as his front tire hit her back tire off into the air he went landed on the left side of his face and his right arm did a funky twist. By the time we got there the VFD (who I truely love) did the original assesment and holding c-spine. The attendent on this call (not me) was doing what ever it was she was doing as took c-spine over. This guy decided that it was a good time to start swearing around and flail his arms around everyone (but me) moved away. I proceeded to tell him to quit swearing at us and informed him that if he continued I would have RCMP arrest him, then in his ear I whispered that if he went to cells and I couldn't continue to help him with his possible neck injury and IF he did have a fracture that he was taking the change of becomming a candiate for a wheel chair and with that the possibility of never haveing another erection again in his life. He settled down right away, wouldn't talk to me but then again I wasn't asking the questions I was the one in control of his neck. Its amazing what will settle a pt down and what will send them into orbit, and the same for any medics. I have been in many situations that have sent me to orbit internally some are Get off my land you white Bi tch I have spit at (Im abit quicker than the spitter) My arms have been raked by finger nails I have been kicked I have had beer bottles and various other things thrown at me There are many other things but not once did I strike or really want to punch the crap out of them. I try to look at the situation of their being and wonder how I would behave in the situation. Alot of time these people are addicts in way or another, homeless, abused in every aspect of the word and the chance that they have started their day with a spouse or parent telling them they are a worthless piece of crap is high. We as a profession really need to understand the circumstances of our pts, use alittle empathy and be professional. To the medic that punched the crap out of restrained pt should be restrained and see what its like to have 0 defence. I dont care if the guy said he slept with his grandmother there is no excuse for his actions.

As this is a terrible accident and the driver has history. I do curse at one thing about motorcyclist is that they are at times hard to see. Lots of them have dark colored bikes and wear black leather. I personally have a hard time seeing them. Now the fact that the biker was trying to pass the vehical says the driver of the car didnt do a look see and should be held full responsibility. Everyone that use the roads are responsible for safe driving. I would like bikers to wear hight vis vests at all times along with helmets.

Ok I am probably right off base here but the statement of unable to pee can be different that unable to create pee. So Im going with a blockage or detachment of the bladder. If you cant get rid of urine all sorts of things can happen.

If there is no pedals manipulate ankle until pulse returns, splint and go. I personally like pillows for splints for ankles. I have never used a sagar for an ankle and to tell ya the truth i cant even think of were you would put the strap. Annie I would like to clarify the $2.00 issue. In my station I get paid the $2 for holding my pager and if the pager goes off I get paid my regular wage for 4 hrs regardless if the call only takes 1. It use to be that I would loose the pager pay if I got a call but for the last year they changed it on their own and gave us all of our pager pay back for days that we recieved a call.(hope that makes sense). There is a min response time but I dont know what it is for sure but 15 min rings a bell. Depending were I am in town it takes no more than 5 min to get to the station. Most people that are on the 2 hr shift are in rual arreas like myself and do have other jobs. I am a very lucky person and my employers allow me to leave my job as a bean counter as long as I make up my time. There is really no reason for us to employ full timers here as there is not enough call volumn to warrent it. Our average about 250-300 (just estimating) and sometimes I do have 3 calls in one day. One thing you will find in arreas like were I live is that people got into BCAS for community service, it use to be totally volenteer when it was started here. They didn't even provide the training, The original medics here and there is only one left that does min shift had to pay for their own training. (Iv been lucky in my timeing as all of my training has been provided for me one way or another) Here as a part time station I am required to available for 3 shifts a month to keep my senority but the new ones that come in the system are required to available for 8. That does not mean that they get all eight. It is unfortunant in some cases as they have a hard time filling up the scedule, even here with me available for 6 day shifts a week and I keep my pager on incase it goes to second page and with about 10 other part timers. One thing I have found with the new ones that were hired and trained is I thought some of the night shift pressure would go away and some did we have less holes than before. The problem is they now dont want to work weekends and holidays, and the employer cannot tell them that they have to work certain shifts. (this senerio is different for the full timers) I hope this answers your questions in my long winded way.

Ok Dust Im going to stop, It was never intended to go this far to make you so upset. But on the topic of the strike the fact is that you should have read my later post that explained why this strike is taking so long and it is again because of a Provincial Election that we had and the government in power is not able to commit to any financial agreements while they are campaining. Also yes the public knows we are on strike but since we still arrive in their time need they dont understand what is really going on. They are going to the table for 2 days of talks this week. Now as for the little dig of why I dont post great thread is because I really dont have the ability to do so. I will admit it I personally like to read a thread and then get my brain in gear and respond and that is just the way it, but I am posting a Thread under funny stuff if you care to go see it as I like to bring humor into this world and I do that pretty good. Hope you understand that many people whether you like it or not took this personaly and not as in you were bashing the ability of the BCAS Paramedics but because you were slamming and laughing at an issue that is beyond our control so try to look at it that way.

ROFL! If you have to remind the public that you've been on strike for two months, your strike is teh FAIL! I did it for the same reason I post every other news article that I post here. To inform. No agenda. But I am certainly as welcome to post my opinions on these stories as anyone else, aren't I? Nope no sarcasim there Who did I judge? I posted a story and said their strike was a failure. That's it. Don't play the innocent victim here. You and your attack dog friend here made it personal, not me Woof Woof I hate to point our the above ROFL! If you have to remind the public that you've been on strike for two months, your strike is teh FAIL! that is exactly what made this personal, you should read the later post I made that explains why this has been going on as long as it has, its actualy makes sense, you'll see. All you have to do to the attack dog friend is rub her tummy. Don't play the innocent victim here. Come on Dust you know me better than that (and annie also) to think that I am innocent in any aspect of my life and victim I am not. Why dont you post the quaility post you use to that actually made us think. PS unlike out resident dog AKA boytoy I have been in the military and come from a very long line of military people so I truely understand the crap (and enjoyment) that lifestyle can be.

Would the managers who havent been on the road for a decade or more (and BCAS is very top heavy btw) train them ... in Masset ? One of the issues is that the full timers as it was told to me was that they are being forced to do overtime and the employer went to court over this because they say if you were available for overtime berfore the strike you should be availabel now and this is to keep up the standard of care. It is also my understanding and I could be wrong that they are scheduling this ot. This is kind by passes the you have to answer the phone to be made t go to work. Now I think the reason for this court justified this is because they can not hire other paramedics to replace the one they have.

JP I just did a safety training on the swine flu and in the midst of it all I recall Toronto EMS has UV in their cars. After the SARS they are the leading medics in infection control as far as I have been told. How true this is I dont know as Im on the other side of the contry. But it is a great idea. -
Okay there is one other important factor in this strike action lasting for 2 months. We also had a provincial election in the past few months and because of that there is no negotiations for anyone, anywhere, With the possibility of a new government the one that is currently in office can not legally make any financial agreements while campaining. Now that the election is over there are 2 days of negotiations schedueld this comming week.

Well Annie you did hit a core since you got the STFU comment. When you are deemed an insential service you only have limited strike actions that are not going comprimise patient care and that is the long and the short of it. It was not poor planning that is making this a long dragged out issue it is the fact that the government has us by the short and curly's. Now to tell you the truth I am not a raving union person but I do support those who are. I believe that if you are a first responder no matter if that is fire, RCMP, or paramedic, it you are on the same scenes and under the same stress then the pay should be equal. I am a part timer and when my pager goes off because of BC Labour laws not the union I get paid for 4 hours even if I work for one. Now that sounds good to most but if I only get one call in a pay period that really isn't much but oh ya I get 2 bucks an hr for holding my beepy thing. When you live in the city and work full time you get paid by the hr and in this day and age it isn't alot, oh ya they dont get the extra 2 bucks. There are safty issues and others that are not really reported on because they dont sell the papers. I bet some people actualy think that we wont show up to their emergency because of what is said in the media. Now to Dust, My response was neither sarcastic nor obtuse. It was honest. I admitted to you that I had no solutions to this situation, and that I lacked the personal motivation to give it much thought. Would you prefer that I BS you and act as if I had some answers? I also went to a university it was called the school of experience and I got in there by graduating from the school of hard knocks. I am just as intelligent as you and in some ways more but on the other side of the coin I can say the same thing about you, you are more intelligent than I am in some things. Read much? What does my occupation say there under my name? RETIRED! I gave all I had to this gong show for thirty-five years. I'm done with licences and certifications. But I've still attended four multi-day educational conferences in the last six months, at my own expense, and have two more scheduled for July. That's a lot more than most working medics with two good legs, and I suspect more than you. When you're sitting on your fat arse in a wheelchair after over three decades of service, through three wars, you can sit in judgement of my commitment to the profession. Until then, you have not earned that privilege, so STFU So one question I have about the above is Why are you still doing the education when you should be out golfing. Oh ya I bet its because you just cant leave this gong show and you have a desire to learn. So you say that because you put in 35 years of service and went through 3 wars that gives you the right to judge, sorry Dust it dosnt. You really do have alot of good points at times and Im sure you have the ability to let otheres know your knowledge but you have to learn how to deliver it without the spars attached. In closing Dust you really do care about issues because if you didn't first you start threads and second you wouldn't respond to our posts. You really do remind me of the Grinch and we all know the ending to that story.

Well I was going to leave this one alone but I will quote Dust "I don't ever anticipate caring enough to come up with such answers" Well you sure care enough to slam the cause and your crappy remarks. It is unfortunate that you choose to make these remarks to an organization that is a rock in a hard place. You really should pull you head out of your butt and maybe think that if you have a good solution to the problem lets hear it you never know you actually might come up with something worth reading.

Well I apperently hit a nerve somewhere. Its not that I dont want to learn or do things for the profession. First my concern is the patient and that I do no harm to them. Basically what I was saying in my long winded way is that if I feel that if I look at the whole scene and think that there is the SLIGHTEST chance that my pt has a spinal concern Im doing it. If I look like a fool doning it so be it. When I did my first training session it was a WCB level 3 and they use to teach you how to rule out a spinal. Now they dont teach that because in a community close by a Dr. asked for x-rays (after he took off the collar because the pt didn't have pain or any other sign) the tech says to the guy hop up on my table and we will take the pic. low and behold the pt screamed and collapsed and now lives in a wheel chair because hey he did have a problem. So if Im wrong from learning from others and being to suspect with this sorry but I do error on the side of caution and there is not one thing wrong with that. And FYI I do rule out Spinal procautions but I dont do it because of a study I do it because of the training Iv recieved that actually shows me how to do it and do it right.
