Personally I really dont care what any study says because in reality it is one persons perception of any issue. They always change,remember when some said milk causes cancer (showing my age on this one), then years later it was proven that it dosn't. For every study that states one thing there is another that states the opposite. When I have decided to do a spinal I will error on the side of caution and I do this because a very dear friend who isn't a medic anymore had a call of a lady who fell on a fish dock. Well this woman showed no signs of a spinal injury and had no pain. She for what ever reason she had a gut feeling and did the full spinal. Good thing the lady had a neck fracture. If you always take a good look at the mechinism of injury, you will have some idea of whether a spinal is needed. Look at the age of the person a fall standing for an 90 yr old is very different than one from a 16 yr old. Do they have any numbness and tingling, well you can have that without pain and they will get a spinal. Spinals are need for many reasons not just car crashes and falls, dont forget the guy who has had the crap beat out of him, or the gun shot wound. Some one said not to spinal because the board is to uncomfortable and the pt will move around anyways, we dont use the board if possible and will utilize the scoop or clam shell and alittle padding goes along way in pt comfort. The KED was also mentioned and as I have said before this is my favorite piece of equipment. I have spinaled a 6 week old baby that fell off a change table to a guy who landed on his feet from a roof. This guy crawled to his truck and drove 30 mins to the hospital just to find out he couldn't get out. The unfortunate thing with the KED is that the pt has to be stable and is usless on the calls that need the quick extration. There are times when I have had men refuse the collar and I wisper in their ear that I dont have to put on the collar if they dont want it but to be aware that if you have a spinal injury and it is agrivated you are taking a chance of your penis not working. I have never written on my form collar refused. I have had Drs without doing any assesment rip off the collar to my horror, then to find out they have a fracture. When this happened a Dr friend of mine was in town and explained that some drs are just like that and if I watch closer I will find other things they do wrong or not up to our standards. It will only take one error for someones life to change so I dont understand why anyone would even take that risk just because some study says.................. Any who I will still spinal reguardless of what some study says because to tell you the truth I can always justify doing so but trying to justify not doing it is alittle harder.

Okay Im saying no on the spreading of the ribs, I know some ambulances are clean but are they that clean and just plain yuck. I am considered a basic but I have done some ALS activities under their license and have enjoyed doing so but I have to say they have done the schooling and paid the big bucks for the schooling so I have to vote No. -

This is one of the reasons people in the State have such a hard time with helping out their neighbours. Everyone wants to sue someone, well not literally but you get my jiff. Why on gods greed earth would you try to sue someone for your own error in judgement. If it is winter then always think there is going to be ice and deal with it. You probably went running into the scene without thinking to slow down because it is winter and there is ICE. Do your scene assessment and if you have to walk then walk. Once you see the oversized patient call for a lift assist from the fire department and before they show up ask a family member there to clear the steps for you. We actually have little spiked slip ons for our boots and they do work but only if you take the time to put them on. Im sorry you got hurt on this call but take your lumps and learn from your mistake and don't try to blame the patient or the family for it.

Well I would work this code as he dosn't fit in my protocol not to. As l don't pronounce death because im not a Dr. and someone is performing CPR thats all I need to make my decision.

Here we go 25km over the limit on a code 3 only if road conditions permit. I as the attendent have told my driver to slow down. One thing we have done in the past is make everyone lay on the cot and be driven around town so that you can see how it feels. That made alot of drivers become aware of what it is like for a patient laying on a cot grabbing for the sissy bar to hold on. It really dosn't pay to go over your limits as in reality you are not getting there any faster. My personal reason for lights and sirens is just to get people out of my way it dosn't mean I have to go like a bat out hell.

Okay Dawyane I guess that I thought that the original poster posted this to show a different side of what can happen in our profession and I was also trying to point out that at times your phycological side dosn't just let you read something like this and say hey this is a good story you have to pick it apart and not the good side. I dont think I misunderstood you as much as I thought when I read your post where is the just human side of you that can take a story and enjoy it. Im not thinking that any one wanted to applaud these guys it was just a feel good story and it has gone arye. Sorry about your poor decision in Kandahar and Im glad it didn't cost you your life or the others that came to get you. Heres another story about dogs in the battle field. My father who retired as a W.O for the PPCLI has done many nato missions from Cypres to Bosnia and he always told me stories about the fact that when he did these missions he always found a dog and shared his food with it. For many years I thought that wow what a great guy he saved a dog while he was there. In fact the only reason he did this was so that the relationship he had with these animals was so they would warn him if someone came near where he was and yes the dogs thought of him as the food supply and did save his ass a few times. Now my father is the type of man that will teach a dog to salute and the cat to die when shot and yes he truely loves his animals. Now one day I asked what ever happened to the dogs when he left his duty, and his answer was he shot them so that they would be put out of their misery because once he left he knew that they would probably starve to death. So when you make the comments of dog behaivor I dont disagree but the story was about the fact we as human beings will help others them being people or animal. I think the emts did what they could to get to their patient and lets face the fact the guy didn't have life threating injuries and the time could be taken to make his best friend more comfortable. One more thing about what can happen to paramedics who loose the reasons why we do this job. I for one do it for my community and it is not my career. The other day we had a station visit from our superintendent and his boss. Now this boss made a statement that made me think if I ever think like this I will quit. He said to us that We are different in remote EMS because we are treating our friends and neighbours. Where as when we encounter a death or a very serious accident we act with our emotions. In the city they will go to the same kind of calls they don't react with emotion because they are not their neighbours and they can go to the next call without as many issues as we in the rual have. Not that what he said is untrue but when I dont care from patients and treat them the same way I would want to be treated is the day I quit. Spenac I hope this dosnt ruin our traitor relationship because that would make me and then i would be ps Dawyane ty on reading most of my posts with an open mind and actually admitting you like at least one lol

Iv been reading this thread on and off for the last while. I found the orginal story very heart warming and thought why would an investigation even be needed. Now to Dawaynes comment. I do agree that our society has humanized our furry friends. This is why we buy our dogs warm coats and feed them better than ourselves at times. The fact that we have decided not to eat our dogs and critisize those countries that do is another clue to this humanization of our pets. You are right Dawayne a dog is a dog and yes if I had to I would have a chomp. From what I can gather from what you wrote you are saying that dogs have no feelings and this is where I disagree. Dogs have and will continue to put there lives at risk for their human companions without being trained to do so. I have been around dog training for my whole life and still have very well trained and behaved dogs. My experience goes from sled dogs, bird dogs, show dogs ( and for those of you that think that isn't training getting any male to stand still and have some stranger grab your testies without snapping can be a challange even in the human world) and just basic manners. My methods have been a combination of Koler to The Monks of new skete and everything in between. I personaly think the fact that you trained dogs for various things is great but one thing you have left out in that statement is that you trained them and correct me if im wrong gave those dogs to other handlers to create a relationship with, and that is what people do with their dogs no matter if they are just a pet or tool that they use for their job. The fact that you critisize the medics for thinking outside of the box is a bit disturbing as not one situation in any part life is the same. Im sure the medics that went to that scene saw a barrier to their patient and did what they need to do and since they couldn't shoot the dog they gave it some meds and they also put some thought into that desicion by asking someone who was qualified to give the proper advise. You also say they spent time fussing to get that information well only on medic has to be on the phone and the other was probably doing his moral and ethical obligation to his human patient remember there are 2 of them. Now if the owner had life threatening injuries I would bet the whole story would be different. I have to agree with Scotty in the fact that you took a good story and tried to find fault in it. The only reason that I think it was posted was to show that there are people out there that do think outside of the box and do have compassion for people and for what people love. I normally enjoy reading what you post as you are a very intellegent person but sorry Dawayne you need to lighten up on some things and just enjoy the things that are posted to make us smile and remind us that we too are human and should have compassion.
