Is mortaility rates the only thing that's important for EMS, or just the easiest thing to measure? If mortaility rates is the only thing that matters, why are paramedics allowed to give medication for pain relief? Pain relief doens't decrease mortaility.

So the obvious solution is to refuse to treat patients on Medicare/Medicaid? Furthermore, how does selling a service to the government require support for any and all taxes? You're point? I don't expect to live off of Bill Gates tit. I own a car. It's a 2003 Sebring with almost 100k miles on it. It's worth a few thousand dollars. It was worth a few thousand dollars last year. It will probably be worth a few thousand dollars next year. However, just because I own a car doesn't mean that I get a few thousand dollars every year because that's what the cars worth. Similarly, I'm a couple hundred dollars in student loan debt right now, thus making my net worth a couple hundred dollars in the red. However, I don't gain a couple hundred dollars in debt every year because my net worth is in the red.

1. Who decides what to do with the tax dollars? 2. What about income taken outside the US? 3. What about charities like the Bill and Melinda Gates Foundation? Do you think if the Gates were only making $1 million a year that their charity would be so successful? Shouldn't someone who makes more than a million dollars be allowed to decide what their money should go to support? How do you suggest the government or society fix crappy parents? Considering that some of the schools with the worst outcomes get some of the most money, are the schools crappy, or is it crappy parents who expect the schools to overcome their crappy parenting? Are there real cost barriers to university education, or is it not every major will produce someone with the skills and knowledge necessary to pay back their tuition? Are all majors created equal, or is there a difference between the hard sciences and, say, ethnic studies or philosophy? Heck, does everyone need to go to college, or do we also need to focus on changing societal views on trades and vocations? I'm in medical school, but I know that I'd never be able to pay for medical school without student loans. I would argue that parents and upbringing has a huge effect both on intellectural capacity and work ethic, especially work ethic. ...because they often have the choice not to buy those things. The classic case study was the 1991 Federal luxury tax, which put a lot of people out of business when the rich decided they didn't want to pay an extra 10% for a new yacht. Who needs a new yacht to survive? Well, it's life or death to the people building and servicing yachts.

In theroy because capital gains shouldn't be a primary source of income. In reality: Stock options. I wouldn't be against treqating stock options style of income different from investments that aren't made as a part of compensation for work performed. After all, non-stock options investments are being made with money that has already been taxed once.

Sure, the same protection everyone else has against being discriminated against due to sexual orientation. It's "sexual orientation" not "everyone not heterosexual." Also, wait a minute. Did the last few paragraphs really just deal with thinking that people have the ability to simply turn off an accent? Challenge to everyone. Speak perfect English. No American accent. No regional accent. No regional slang. Hell, given the fact that accents are all about shifting specific sounds around and using slang, I doubt anyone -can't- speak without an accent. In fact, I'll bet that most people don't even realize that they are speaking with an accent when they speak.

When to believe the pulse oxymeter, when not?

1. Displayed pulse matches manually measured pulse. Clinical correlation. If there's a waveform on the pulse ox, then the waveform is regular and not hitting the upper or lower limits. 2. Lack of clinical correlation. "Oh, look, the patient is struggling to breath, but hey, there is an SpO2 of 99%, so I guess they don't need oxygen." In general, I think most erroneous use of SpO2 readings is user error either in misinterpreting a reading or using the device in a way where they reduce the chance of figuring out that the reading is bad (i.e. not verifying the pulse rate). 3. What other point of care testing can we use to titrate treatment, or do we just go with a NRB for everyone? -

Sort of like how masculine pronouns are also the neuter pronouns, it seems like "gay" is both masculine homosexuality and neuter homosexual whereas feminine homosexuality has it's own term (lesbian). 2. As I said before, public displays of over affection repulses me regardless of if it's 2 men, 2 women, or a man and a women. If 2 men or 2 women kissed each other on the lips in a "Hi/Bye my love" type quick kiss, I probably wouldn't give two glances. 3. Girl on girl is acceptable? Let's be honest here, porn lesbians look nothing like most women, little less most lesbian couples. The stereotypical sexual/porn lesbian is going to look more like Alison Brie. The stereotypical real lesbian is more like Rosie O'Donnell (and I'd use Rosie as the example even if she was straight). I'm sure most guys here would love to have two Alison Bries going at each other. Two Rosie O'Donnells? Umm, not so much, 4. Why is lesbian porn OK, but male gay porn not? Power. Consider historically the views of power between the sexes and the power relationship between receptive and giving persons? Heck, why is it more of a big deal for a man to be a nurse or receptionist than a woman being a physician? Why is it that the only acceptable skirt for men (kilts) is almost fervently defended as "not a skirt"? Why? Because society still views being feminine as weak and bad, thus when women cross dress no one cares because the woman is taking on power. Men cross dressing, however, results in an almost visceral revulsion.

So, is someone with androgen insensitivity disorder male or female? They have a vagina, but also have testicles. Does it matter what they, as an individual identify as? Does it matter that they're male on a chromosomal basis?

Yet medicine takes a much less binary view on sex, as well as gender. Is medicine not reality? More importantly, in my opinion, is gender, which is defined more by society's expectations. However, those expectations change. Constantly. It wasn't too long ago that females wearing pants would be completely taboo, yet it is now, based on those values, expected that females cross dress.

Gay pride day is stupid. Heterosexual pride day is stupid. If sexual orientation is innate, and thus not a choice, what is there to be proud about? Note: absence of pride, and presence of shame are not the same thing. You can be something and be neither prideful or ashamed of it, especially if it isn't a choice.

In regards to cruelty in animals, how can we even begin to know? The last time I checked, we haven't cracked the code of barks or meows, or grunts, or other non-human animal vocalizations yet. Since cruelty requires intent, how can we say that the cat playing with a dying mousing is or is not doing so to be cruel? Sometimes I'm amazed at the hubris of Homo sapiens.

Given the debates? Ron Paul. However I'm forgetting myself. When I was growing up being Republican or conservative meant something besides pandering to the military industrial complex and Christian fanatics. You know, constitution, freedom, small government. All those pesky things that it seems that mainstream Republican candidates only treat as an annoyance.

They can still go after the estate. The question is, do they want to go through the effort and the press reports after they sell the debt to a collections agency or start doing things like putting a lien on the assets of the estate? I'm not arguing that insurance isn't easier, and that at least Medicare requires transport (your money may vary with the other insurance companies). Just that it isn't the only way to bill for calls. -

Here's the problem I have with transporting because of H and Ts. Even if transport is immediate, how long has the patient been down prior to arrival? Now add on scene time (assessment, initial management, packaging, moving to ambulance), transport time, transfer of care (ambulance to bay, patient to gurney, report), and initial assessment. Now how viable is the patient who has been in arrest that entire time?
