  1. Orrville is around 8.5k per Wikipedia. You know what, if they've been doing civilian EMS since EMS was first conceived, doing it well, and doing it professionally, what's wrong with it? Is it any different than the medical transport/911 hybrids that are currently popular? I can think of worse ways to staff an ambulance than having it contracted from a company that also runs a funeral home.
  2. Quick question, would the fact that this would be the only non-physician board in the ABMS cause undue friction? Would it be more feasible for the eventual spin off from ABEM be independent instead?
  3. In Texas, though, there are two levels of paramedic. The "Licensed Paramedic" has a degree while the "EMT-Paramedic" does not.

  8. D50 is definitive treatment for hypoglycemia. Now, sure, the patient might need observation and a good meal after their sugar is raised, but then that's treatment for diabetes, not hypoglycemia.
  9. You know what's really funny? The same people who like to think that paramedics are the same as a nurse or RT are the same people who run when ever given a tough choice or a pat on the back. Too many paramedics want all of the toys and none of the responsibility.
  10. Well, patients who have ROSC are no longer in arrest, correct? Is it not true that the longer a patient is in VF/VT the harder it will be get ROSC? If the patient is in a shockable rhythm how can you justify not defibrillating the patient prior to moving the patient into the hospital?
  11. Ahh, but what IS definitive care in a VF/VT arrest? More importantly, for those two rhythms, can you provide definitive care? I know I can. As a basic who's admittedly never been in this situation, I'm going to have to go with the BLS care and defibrillator if indicated before moving across the ambulance bay and across the hall into the code room crowd.

  14. A fire engine on an EMS call in So Cal (ESPECIALLY in Orange County?). Say it ain't so. At least this wasn't OCFA in Irvine who sends an engine and a fire department ambulance to every EMS call even though they use a private company to transport.

  17. I think that there are better ways to go about releasing information. An example from So. Cal. is one of the larger ambulance companies (Care Ambulance) posts reports of their strike teams when ever they are deployed or any MCI style situation.   19. 2 words. Huntington Beach. I grew up in the next city over and, to tell the truth, I wouldn't mind if HB fell into the ocean tomorrow. (Besides, that means that my old house would be ocean front property now). Edit: Did the firemedic get a med com order or an AMA? I don't recall Orange County having a protocol to refuse transport.

  23. I think the closest that your going to get to a randomized study is the Hauswald[sup:707404d928]1[/sup:707404d928] study. This was the one in 1998 that compared trauma patients going to the University of New Mexico (all had c-spine precautions) with those that went to a hospital in Malaysia (where c-spine precautions is virtually unheard of). There was a slightly higher level of neurological deficit in the patients who received c-spine precautions.   25. Just remember, when seconds count the police are minutes away. Besides, per the US court system, the police do not have any specific duty to protect the citizens. See Balistreri v. Pacifica Police Department and Warren v District of Columbia for case law.
