I am interested in why you believe that volunteers should be held to a different standard than paid personnel. Don't patients require the best care possible regardless of if the person is volunteering or not? -

I'll be honest a bit here. I'm not really big on back boarding every SNF resident that has a fall. That's because most of the time they've been moved drastically (walking around, moved back to wheel chair, etc). That said, unmoved patients who have a history ALOC/confusion and patients with complaints (neck/back pain, LOC/neuro changes) will be boarded, properly, everytime. Assuming that the crew is willing to get touchy-feely with their assessment (which it looks like they didn't), I don't honestly feel that every fall needs to come in with their clothes cut to ribbons. This said, if you are going to back board, do it properly. Especially since if you're going to be back boarding, you can also 'borrow' a sheet from the SNF and sling the hip as well.

Where did that one study go that stated that skill retention and decision making ability went down as the number of medics went up? Why do you need ALS engines if you don't hold the certificate of necessity to begin with? Can we say, "money grab?"

I'm surprised that no one has hit the supply vs demand answer yet. Simply put, a lot more people want to be an EMT-B than want to be a CNA. A high demand with a low supply means higher wages.

Well, my Mom was hospitalized for almost a year growing up after she had two strokes. When she had the first one, her airway swelled shut and the ER physician ended up having to trach her (EMS doesn't have surgical airways. According to people who were at the event she was helping to run, EMS arrived and left within 5 minutes of being called). Well, she was never put into soft restraints. When she started coming around that night in the ICU, she wasn't really pleased that she had a tube going into her neck and successfully removed it. That earned her the chance to get discharged almost a year later following rehab with a shiny metal airway still in place. Eventually, she was able to have constructive surgery to fix her trachea.

Dear SNFs and assisted living facilities. If you're going to call us for a transport for your patient who slipped and fell, please do not move the patient unless you absolutly have too. Mind you, it does save us a lot of work because the majority of what we would do becomes just about useless if the patient moves alot, but this is against current patient care standards. Love, JPINFV. PS Your 65 year old resident who is A/O x4, remembers the slip completely, and is now sitting in the dinning room enjoying dinner with her friends probably does not need to visit the ER.

Well, that's not entirly accurate because all of the private services around here provide IFT, including the ones that hold 911 contracts. -

IFT means "interfacility transport." I do believe that other locations call it "NETs" (non-emergent transports). -

Hey now, it looks like our paramedics might finally be able to give aspirin starting sometime this year. Besides, how often are the family actually at the nursing home when the patient starts going down hill?

As with any thread on DNRs, it all comes down to your local policies regarding them. Where I worked, family could, provided that the patient was incapacitated, could resend a DNR at any time, regardless on if they were the POA:HC or not. Similarly, immediate family could request that EMS providers "withdraw or withhold" resuscitation also, effectively making the patient a DNR. There was a documentation policy clearly set forth on what needed to be documented and who needed to sign the PCR as well. Talking about the logistics on who can do what, when with a DNR on an internet forum is next to useless outside of discussing theory and best practices. Any application will ultimately come from what your medical director wants you to do in these situations.

Well, a DNR is not the same as "Do Not Treat." There are legitmate reasons for someone to have a DNR and either need emergency treatment (problems not relating to why they have a DNR. Hypoglycemia could be one example) or transport to a hospital for further care. Granted, the latter should be arranged though means other than the 911 system, but the reasons are there.

Those in glass houses should not throw stones. Since, apparently, you wish to continue with this "Epic Thread of Failure" . Issue 1, stop being a hypocrite. First, YOU posted this thread by your own will. People will respond to it humorously in the spirit of the original post. Just because the mirror is being turned around doesn't make the posts bad. IF you are going to throw mud (even humorous mud), you HAVE to expect that people are going to throw back in a similar spirit. Furthermore, I have now received 2 personal messages from you (one from this thread and one from another). While I do believe that PMs should be just that, private, I do take issue with you calling out other people for using the same tactic that YOU also resort to. If people are never supposed to resort to private messages, why are you resorting to private messages? Issue 2, the victim card. Honestly, I could not care less about your home situation. Unless you are a widow, I couldn't honestly care less that you are a single mother of 3. Similarly, having to work hard doesn't make it so that you're automatically right. This forum does not reside under your roof. Furthermore, if you aren't receiving any child support from who ever fathered your children then something is wrong. Next, the "man" is not holding you down, nor do I honestly believe that working in a "man's" world has anything to do with your situation. I'm sorry, but I honestly don't buy into that glass ceiling pile of bull for 99% of the supposed "discrimination" cases. There are way too many other factors that I can think of that would legitimately affect a person's promotion and pay scale. Yes, child birth is one of them, but that is more a fault of biology than anything else. If you want someone to blame, blame God. That said, according to the movie Dogma, God is a woman, so go figure. Issue 3: No one honestly cares that you got a gold star on one of your certification tests. The supply girl at my old company was an EMT. She worked the supply room because she was to stupid (stupidity and ignorance are different, for the record) to work an interfacility transport job. She also liked to put her certificates (CE certificates, NREMT-B diploma thingie, etc) up on the wall in the supply room. Guess what, no one cared that she passed some CE course or passed the national registry, just like no one really cares that you got a gold star. Finally, if losing your coworkers is such an emotional stressor that you're going to get THIS emotional on an interweb message chat board thingamajig, then you need professional help. Honestly, if losing your coworkers is a cause for these types of replies (and remember, you brought it up), then do everyone, including your children, a favor and seek professional counseling. -
Dust, either your exaggerating the potential and magnitude of the lawsuit or I need to modify my retirement plan to: 1. Place parents in any number of So. Cal's not so fine nursing establishment. 2. Wait for lapse in care (it's a question of when, not if). 3. Sue 4. Retire Problems with having proper DNR paperwork is all to common from both the hospice agencies and the nursing homes.
