JB: I love your energy and science based critical thinking, wecome to the boards. You will find that other members here will take you more serious if you can cite references for your claims. Don't bother citing Wiki, or trade magazines. Good published studies, or articles from credible sourses like Medscape will give you alot of clout here. You will also learn alot from tracking down the research, and in some cases..... find that you were wrong all along. I am especially interested in studies that support this statement, as I have always done it because everyone else does. I have never really seen evidence that it is indeed a supported treatment.

You know, I file this under the same category as "An obs pt preffers a female" To be honest, I have dealt with plenty of obs... very few rapes, lots of domestic abuse, and to be honest, none of them have ever indicated that they would prefer a female practitioner. To be honest, most of the female rape/abuse victims are somewhat comforted by having a male with an authorative uniform in thier presents. The obs patients are always receptive of me, especially when I speak of my own 3 children. The very few times I have had too check for crowning, there was no hesitation from the mother. Anyone who has seen a woman give birth knows quite well, the last thing they are worried about while pushing a human out thier vagina, is if the person controlling things down below is getting some sort of nasty thrill. -

I do think it is discrimination, unfortunatly it is all too common in my experience. -
I was able to do a little more followup with this case. The hospital administered 2 more units of matched blood after my departure, and kept the pt alive while further discussions took place with the recieving facility. The ultimate decision was made that this pt was not a surgical candidate and he died about 4hrs later with his daughter at his side. I suppose he would have survived the 2 hr trip since he did live another 4 hrs after I left, however he would have been recieved in the city hospital only to be rejected from surgery, and die alone 2hrs later. I feel pretty good about my choices. Thanks for your imput all!

Super stoked to see you back man!! You have indeed pissed me off a time or two, but you have also challenged me both intellectualy and morally which had a big hand in making me the professional I am today. I can understand your longing for diagnosis and treatment, and expect you will be posting in the scenario forum so we can get your perspective and expertise in reviewing our real life calls. I for one do not have many Paramedics with any amount of experience to "bounce calls" off of, so I am looking forward to what you offer again.

As you know Timmy, I am not one to hold hands and make you feel better, so I will keep this short, sweet, and direct. Manipulating a fx on a peds pt is very risky especially near a joint. I do not need to talk to you about growth plates, that I know... If what you presented is accurate: These medics were indeed negligent (Duty to act, breech of duty, possible adverse outcome because of the breech). If there was in fact just a partial Fx as the medic suggested, then by thier own admission they completed it by manipulating it. You sir, have a duty to report incompetence! If you let shit like this slide on a peds patient, where do you draw the line? Do the right thing Just one mans opinion....

I'll chime in too. Here is a different perspective. At many scenes, the co-responders do not even realize that I am in charge untill I see something I need to intervene on. A good example is breaking glass, right before a FF breaks glass, I will find the one guy with his visor still up and remind him to lower it. If everything is going smoothly, and everyone is doing the right things.... there is no need to micro-manage. If everyone is trained properly and follows predetermined orders, then the scene commander should not really have to "show authority". A great passive way to show authority, is to be the one too do a good primary assessment of the pt and report back to FF command. When you turn to the fire Cpt and tell him the pt is stable, but you are worried about a spinal injury, so extrication should be slow and careful, you have established ultimate command. How they extricate is thier deal.... as long as it is safe and continuous, you don't need to dictate all aspects of the procedure. As to stopping on scene.... I am that a**hole Paramedic everyone hates, when I pull up and am met by a "nurse" or "Paramedic", or even "Dr." all I really want to know from them is if they witnessed the crash, if so, the speed involved. No offence but "Nurse" means nothing.... Home care nurse? Nurse at a hair transplant clinic? LPN? RN? I have no way of knowing the quality of provider, so I only want to know the results of MY assessment, not yours. Even a Dr, is that Dr. of dentistry? Podiatrist? ER? retired GP? Ahh... nevermind, I will just assess the pt's myself. An old boss unrelated to EMS once said to me "A good manager will train his staff so well, his job will become obsolete"

You are totally right on with this one. The glasses do not make the practitioner! Name brands are strictly for coolness factor, I used hardware store eye protection for my first 5 yrs in EMS. Here are the ones I used to buy in bulk, since the arms regularily break off. Cheapies

If you are just looking for day to day regular call protection, I use the Oakley Flak Jacket XLJ. You can get tinted interchangable lenses for them too. If you go to a motorcyle gear store, they have lots of clear lens glasses at cheaper prices too.

If he doesn't want any further communication, why push it? What do you gain by "explaining yourself"? To be honest, in this post you seem to be desperate for someones approval who does not approve of you. *Note: This is why I NEVER... EVER.... EVER post things on FB that are even remotely related to work. If you want to handle this professionaly, go delete your entire wall to ensure you got the one that upset him, then never post work stuff again. The other option is to continue giving power to this individual to disrupt your life so much, you must reach out to strangers in a desperate attempt to "settle the score" BTW: Welcome to the City Glad too see so many new users being post-active lately.

To be honest I did not go to school, nor live in a town with any african americans my age so I never even had the oportunity prior to entering a relationship with her. But I gotta thank you man.... This is just another piece of amunition for me to use in my argument for a minage trois BTW; How does this work for us in the rural great white north? Honestly, there may be 1 african american for every 10,000 whites. Have you ever dated/made love to a first nations? If not does that make you racist against them? YOU (african american) are assuming we all have "free reign" to date an african american, not aknowleging the fact that african americans are not populated in all states/provinces/regions. What was your definition of racist again? As a man who has standards and self respect, I will reply a big HELL NO! BTW; what does fattest mean?

That's the spirit! Just an FYI: It takes a little under 4 yrs to become a Paramedic in Canada. About the same, or even more in Australia. Sorry, gotta pick on you here; Please check out the last scenario I posted and rethink the above statement. I fully realize that you are just parroting this from other providers out there so don't take this personal. This "EMS is young" mantra is absolute rubbish. EMS WAS young in the 80's! Now EMS is a full grown adult that must be accountable for itself just like any other profession. Blaming corruption, ignorance, greed, and misdirection on the fact that the profession is "Only" 30yrs+ old is a crutch used by providers and management to continue a cycle of unaccountability and lazyness. -

I believe all EMS personnel should be required to get at least 6 credits in A&P, and a basic english course including medical terminology and introduction to research. Martial arts is not on the list. If you are in a position on a regular basis that you need to protect yourself physically, you FAIL at basic scene safety. BTW: Why do you capitalize every word?
