His temp was fine 38 C. I’m not qualified to do a BGL. The day is overcast, not to hot, not to cold. Skin is flushed, clammy and sweaty. I forgot to mention his pulse was Tachy and irregular. It’s not cancer or anything exercise induced. Let’s keep in mind that this guy is a well respected member of the football club and might ‘withhold’ certain information in order to protect his reputation. Spent 2 days in hospital with a cardiac review, is now playing football again.

18 year old male (football player). Perfectly fit, healthy and well liked. Time In - 1545 Players walks from football field after 30mins of playing C/O generally feeling unwell and nauseous. Play then vomited; states that he feels good as new. After a mouth full of water and a 5 min rest he returns to play. Time – 1722: Games has finished. Player collapse on floor of change rooms. LOC for approx 45 secs, while unresponsive player began shaking, it wasn’t like a seizure or anything more like when someone has a fever. Player regains Consciousness. C/O – presenting with: -Pins and needles/Loss of sensation in arms and feet -Sever nausea -Severe Dizziness -Dry mouth -Dysphagia -Diaphoretic -Abdo pain -Increased abdo pain on palpation -Feeling drowsy and wanting to sleep -Feeling off balance -Feeling very weak -Feeling very hot -Skin very clammy Vitals: -Tachy 135 -Resp 17 -BP 140/95 -PEARL -Motor reflex action was a bit slow when I did the pen to the foot -No PHx, NKDA and Nill meds. -Last eat a chocolate bar for breakfast, drunk a 500ml electrolyte energy drink and about the same of water. Nothing since. No sign of any injury. No bleeding. No cervical pain. No hx of trauma or blows to the head or anywhere else. From the player has told me there is nothing to suggest his symptoms. **Refuses any ambulance transport or for me to even call one, sings the refusal form** He was quiet aware of what was happing around him and was able to answer any questions I asked so I couldn’t over ride his decision. 1730 - Player vomited, felt fine. Got up and walked around, no symptoms. Maybe 2 mins later he collapsed this time with no LOC – “legs gave way” Symptoms returned. In true football sprit and I really didn’t know what else to do we bundled him into the car for a 15 min trip to the ED. He went LOC for about a minute half way through- Awoke after sternum rub. Arrived at ED - LOC again this time for about 5mins. Nurse had no idea how to get him outta the car so well called the paramedics and got him inside. What’s your diagnosis and treatment?

Hey rdenman26. I’ve just spoken to a mate of mine who is a SAP with QAS. He has worked with a few English paramedics. He says you will walk straight into a basic care paramedic job. Whilst doing this you will have to complete certain packages and have your skills tested which will make you an ACP which gives you your I.V drugs and 12 lead. That’s about all the information I got from him. ICP can perform cardioversion. In regards to the relocation assistance this what he said “I’m not 100% sure about relocation expenses but I’m sure they would cover much of the cost (pretty tight if they didn't). you can do the uni upgrade course at QUT chargeable to the QAS.” That’s about all. The best people to contact would be QAS on there recruitment email address with your questions. Sorry I couldn’t help more.

When I was about 14 or 15 that must have been the prime of my whackerhood. But comeon how many 15 yo do you know that have a professional looking photo ID badge that has ambulance written on it or flashy new uniforms covered with the word ambulance or a walkie talky! It’s every kids dream, everyone at school was soooo jealous. I remember taking a fluro yellow reflective tabard that had ‘medical’ written on it to a party. Well it ended up being taken off me because everyone was fighting over it. Then they rang my Mum to come pick it up. I got in so much trouble lol. Now everything stays in my room unless I’m on duty. I don’t even mention I’m in EMS unless someone asks. If I’m with my mates or at a party I get really embarrassed when someone gets hurt so most of the time I don’t even pay attention and hope nobody notices me. Once I pushed the test button on my fire pager to make it look like we had a fire call to get out of Biology then walked up the street. I have a massive whacker pack and just got a big resus bag (only had to pay for the reg) because my football club is to skint to buy proper medical gear or when I do a sport medical coverage by myself. Apart from it being illegal in Australia to have lights in your POV I don’t think I would anyway. How embarrassing! Not to mention a major turn off for picking up chicks! ‘hey, look at my fully sick car, it has flashing lights. Do you wana hear my siren?’ And I don’t get my license till October. So hopefully when I get into the real thing I’ll be whacker free!

I’m so over this debate between BLS and ALS! But I’ll add my 10 cents because I’m board. I CAN NOT believe that EMT B work on Emergency Ambulances! I almost fell flat on my face when I found that I have a better scope of practice than an American EMT but yet I’m not allowed near an emergency ambulance which to me seems logical! My correct title in Australia is Advanced First Aider which is a Cert 4 in Emergency Response. The course I have done covers everything from bandaid patrol to bumps and bruises, fractures and splinting to AED, medications administration, Pharmacology, Intravenous cannulation and fluid therapy and even wound closure. I can only work with a private standby service or as an Industrial First Aider. If s#%t hits the fan or someone needs a ride to hospital then I call 000 and get the ALS guys to come help me out and take over care. Now if I worked on an emergency ambulance with the training I have received I would crap my self. I mean if someone’s having a cardiac episode what more can I do than give them some GTN or aspirin, 02, get the defib ready and call 000. If I was having a heart attack I would be more freaked out that the people who rocked up could only put me on 02 and race me to hospital. At least when the ALS or even better MICA guys rock up they can pump me with drugs, do a 12 lead and hopefully get me pretty well stabilised before I’m even off the floor. Personally I hate with a passion that I can’t do more for my patients but I’ll get there one day. I plan on continuing my education because I could think of nothing worse than being stuck doing BLS for the rest of my life. Frankly I don’t think I could handle this job much longer without knowing that I wouldn’t be able to do more or couldn’t continue to advance my education. It seriously scares me. I totally understand why some people like BLS. I work with people who LOVE getting out there giving out bandaids and ice packs but when it comes to an emergency they can handle it but would rather be some place else. That’s cool, there only first aid that’s all people expect from them. They call for help and treat the patient to the best of there ability but ALS is only a phone call away. They know when to call for help and when to back off. Not everyone is cut out to be ALS but working on an emerg ambo is a bit far.

The thing I hate about our ALS medics here is fair enough they have their flashy new shinny Bachelor degree but they can’t communicate with people or relate to them! There just like a needle pocking drug giving machine. Ask the patient questions that only relate to the injury. What’s wrong with changing the subject for a bit? Making someone laugh? Lighting up the place? It’s scary enough having strange people you don’t know all around you doing strange things, pocking and prodding. There like bloody drones!

Hehe that’s what I hated when I was a metro cadet. Adults took the whole child protection thing way to far. Oh quick, cover little Johnnys ears the patients going to swear. Personally I don’t swear that much and really don’t care if people swear around me. As 334 said being around school or even the local football club, swearing is such a common thing, it’s almost like second nature. I recently attended a kids dance party were I had an 8 yo male w/ a # neck of femur w/ possible internal bleeding after he stupidly jumped off a wall (that’s right a wall lol) Well lets just say this family was from a lower socio economic background, the kid had 2 last names because he didn’t know who is dad was. I rocked up, started working on the lill tacker then his sister appeared out of the woodwork. OMG!!! The crap that was coming outta her mouth! And it got even worse when I cut down his jeans! The security ended up pinning her to the ground as I had to back off while she was trying to kick me in the head. Well sorry love but I think his leg’s a tad more important than a already torn pair of $10 jeans. When the paramedics rocked up and we were loading him into the truck the ‘mob’ arrived. Well out jumps grandma (with less desirable teeth) and 3 car loads of kids of which most had alcoholic beverages in there hands. They were demanding to know why they hadn’t been contacted and why we hadn’t seeked permission from them to the paramedics. So here’s me sweating under my collar think oh great there gana sue me for breach of care. But lucky as it pans out the event organizer had tried to make contact with mum after the older brother had supplied a number but there was not answer. So we were in the clear. But I’ve never, ever heard such crap coming from a 15 year old girl. It’s the same when you’re treating an OD, in the middle of a pub brawl or treating an assault patient. Learn to block it out or don’t go into those types of situations. I really should get back to homework now

Insight ay? Well it pumps air into the patient Nahh, I’m not sure what you mean? I hate vents to! My mum recently had some surgery done on her hand and the bloke next to us was on a vent, I hated it! The noise is horrible! The machine is so big and has so many buttons, screens, noises ect… When I did my EMT course they sorta touched on vents but didn’t really explain it in much detail. It was a little red box with a little screen and a hose. They pretty much said the IC medic intubated the patient, they’d calibrate to how ever much 02 the patient needed and the only time we could touch it was if they arrested, then we have permission to rip off the hose and clip on a BVM.

I would agree with Bushy on the motocross. Motocross riders are the softest group of daddy lill boys / show pony’s I’ve ever meet! I mean a broken leg is the worse thing that could happen and requires immediate emergency traumatical attention, right? Seriously, you gotta love it when they turn around and say “why did this happen to me”? Like good lord! You only signed a form that said serious injury or death can occur while racing, at riders briefing they told you that motocross is a dangerous and possibly fatal sport and I hate to break it to yah but motocross is an extreme sport and you still turn around and give me the ‘I expect you to be sympathetic talk because I broke my leg and I’m horrified that I got injured because I’m hardcore and never get hurt ’. Oh and ohno! Your can’t race your next race… Hate to break it to yah buddy but your gana miss a few more. Then you have the nerve to tell me that the penthrane does nothing for the pain but you’re bombing out on the damn bed and giggling like a lill girl, stop lying! As for your little fake Barbie girl you call a girlfriend. Don’t think she’s getting in the truck with us! All she’s gana say “ewww omfg why is his leg like on that weird like angle” Well love, that’s generally what happens when you brake a leg. Get a life, get a real girlfriend, harden up and play a real sport!

I HATE!!!! The following… When patients ask how old I am or how old I was when I started… It’s so not reassuring. Or when members say ‘I’ll get Tim, his one of our more experienced members’ But how can a 17 year old be experienced? New years eve I had a heavily intoxicated young lady projectile vomit down my front. She then came over in a pool of tears, gave me a big hug and mooshed it in even more!

Ok, I know this is a stupid stupid question but talking with some weird guy last night who had recently undertaken advanced first aid with some shonky company I’ve never heard of. He went on to tell me that the instructor had taught them to use tin foil and a car battery as a Defib. The thing that got me was he actually knew what he was talking about. Explained how you put the foil of the chest and position the 2 wires. Just the way in which he confidently said it made me think that they actually did teach him this. On further investigation and to rule out any further suspicions, I googled it. To my surprise there is a website that has information on this. I can’t say I’d ever try it nor have I heard of it till now. Has anyone else heard of this?

That ALS can administer better pain relief to kids under 14 with out having MICA in attendance! Methoxy don’t cut it sometimes. Other than that I’m happy.
