Wow America has so many medical type shows. In Australia we only really have All Saints (Soap) Medical Emergency (Documentary) Help (Documentary) Sometimes they play ER and Third Watch. Were I live there is currently no medical shows They even took Blue Heelers (Cop Show) off the air after years and years of production and is was Australians number one drama, stupid TV people!

In Victoria we have 2 services. Metropolitan Ambulance Service (MAS) and Rural Ambulance Victoria (RAV). MAS primarily covers Melbourne and RAV covers the rest of the state. As far as education goes we only have ASL and Intensive Care Paramedics which is at least a 3 year degree, we also have CERT & ACO volunteers in remote area were there is a low work load, theses services are also run under RAV. As far as I know there is no medical direction to administer drugs (not from what I’ve seen anyway) if your trained to give the drug then you give it’s up to you nor have I ever heard of a medical control…. As I said this is only for Victorian Services. Some websites. www.mas.vic.gov.au (Metro) www.rav.vic.gov.au (Rural) www.rav.vic.gov.au/cpg_disclaimer.asp (rural scope of practise) www.med.monash.edu.au/mucaps (University/info about overseas medics) http://www.aremt.com.au These guys will be able to tell you if your qualifications can be used over here. They should be able to help heaps.

Handling Bariatric (Morbidly Obese) Patients
Timmy replied to Richard B the EMT's topic in General EMS Discussion
emtpsaveu911: Well what can I say… The obese patient I attended was treated in an extremely professional and timely manner taking into consideration the circumstances that presented. At no time did myself nor any of the crew ever criticize or make fun of the patient under our care in any way. We did our best to help this person in an effort to save there life. That patient received the same, if not more care than any other patient. The crew I was working with even went above and beyond the call of duty, my crew was only the standby team so once the paramedics arrived and a handover was given we have no legal responsibly to stay with this patient, but we did. We even helped transport this patient to hospital which most certainly is not a volunteer first aid teams reasonability. The comment I made in a previous post was out of twisted humour/sarcasm and I am deeply sorry if this has offended you in any way. Just for the record I would never do such a thing, it is cruel and inhumane. Immature maybe, but I do not claim to be professional, just not yet anyway. -
Well you'd think that... Apartly arguing with other officers and digging thought the Hi-Vis bag and getting out the commander vest is more important than saving someone’s life. Everyone NEEDS to know who’s running the scene! lol!.... You wouldn’t believe all the bitching and back slashing they all do! I worked as a comms person at a major event. The 'command bus' is in a different location from the first aid posts. Someone collapsed literally 6 meters from the bus and they made me call a response team to attend because it wasn’t our job to treat and everyone on the bus was non-operational (admin)! So this poor patient had to wait 2 minutes while we stared out the window at them. But hey the command team though it was a job well done because they got to see there ambulance with the lights on pushing its way though the crowd, ohh and they also thanked the response team for such a quick on scene time! What a joke!

I pose a question to you all: Is rank in a volunteer organization necessary?? My volley service is a first aid (BLS) military based, we have sergeants, corporals, officers, superintendents, commissioners, acting officers, discipline officers, officer of fleet, officer of training, team leaders, officer of operations, staff officers, regional officers you name it, there’s a rank. Most of these people who hold a ‘rank’ are complete tossers, I agree someone should be the ‘boss’ but it’s completely unnecessary to have all theses ranks. I have not heard of any other volley service that have ‘ranks’. Once theses members do there little NCO and Officer Courses, they get stripes and pips on their epaulettes and this seems to go to their head! Nice level headed people turn into complete moronic ***holes! When we go out on duty (standbys) these particular individuals feel the need to wear ‘COMMANDER’ or ‘TEAM LEADER’ or ‘LIAISON’ written in nice big fluorescent yellow words on there reflective vests just so you know who they are. These people who wear COMMANDER on there vest have no more qualifications than the next person, apart from the little bit of leadership and event management they may have picked up on these waste of time NCO courses. But yet they walk around with the ‘look at me, get out of my way attitude’. It is quiet embarrassing being on duty with these people as it does looks stupid to the public eye and doesn’t do anything for our image as a first aid provider. To make the situation a whole lot ‘sadder’ there are groups of members (privates, NCOs) who ‘get off’ over theses people, they flow them like a pack of little girls, can’t make a decision for them selves, you know the type? They also run around acting like their ‘paramedics’ some first aid officers even run around telling people their ‘paramedics’ and that our service is a ‘paramedical’, which couldn’t be further from the truth! They also insist on ordering around Doctors and unfortunately if the doctor isn’t the same rank or lower then they must do what they say… When I say this I mean a doctor still over ranks the officer treatment wise but anything else. What do you think of this and how do you suggest my service put its act together?

Guys lets face it, I bet an overwhelming majority of hospitals all over the world have these very same problems. There will always be nursing shortages, more patients, not enough money, increased work load, hypochondriacs ect ect ect but in reality there’s not much we can do about it. This problem has faced health care systems for many years and still nothing has been done about it and I’m sure nothing will be done about it in the future. Sure it’s hard on us and hard on the patients but we must learn to deal with it. Take my local hospital, a rural acute care 19, 5 surgical and 3 ED beds. Day shift has 5 nurses, afternoon 3 nurses and night 2 nurses. Friday is surgical day so they have extra staff on. Now when I was on clinical placement it was madness. 5 nurses to run 19 beds and ED. Yeah sure it was stressful but we copped, patient care was never questioned, I know it’s different to the big trauma centers and everything but we need to make do with what we’ve got and I’m sure if it was a massive problem then more people would carry on about it and possibly something may be done. If hospitals want to send in fake patents or what ever who am I to stand in there way. The next question we should ask is, they’ve got there data, would they actually make any changes to the ED or just sit there and talk about making things better like they normally do?? But I do agree, kind of a dumb idea.

Awwwww don’t me so mean! They have a problem… -
My service has HEAPS of events over new years: Robbie Williams: 50,000 Crowd New Years Eve: They run first aid posts all over the Melbourne CBD dealing with all those drunks and bashings. Pyramid Rock: 20,000 Crowd Summerdayze: Same again Tribadelic: Drug Fest 10,000 people stoned out of there brains! As well as a lot of more local events. These events are normally staffed with at least 60+ First Aid staff, doctors, paramedics, nurses. Then we have the Big Day Out lol This is a major outdoors music event! About 90,000 people attend in extreme heat and there normally pack in there like sardines! We run a Hospital and 4 first aid posts. About 90 first aid staff with doctors, nurses and paramedics, Trauma teams and what ever else they can get there hand on... This years line up! Muse, Jet, Eskimo Joe, The Streets, The Killers, John Butler Trio, My Chemical Romance, Evermore, Something for Kate, The Butterfly Effect ! 8) Don’t you wish you could come!!! As well as ambulances we use these to get around on:

Well I'd call the paramedics and let them deal with it; once they arrive I'd run around the corner and laugh at this poor pts misfortune and the facial expression the medics are giving each other... Joking... This actually happened to us last year at a speed boat racing event. An extremely over weight patient was C/O chest pain, medics stabilised her, our stretchers can only hold about 160kg, patient was well over this. We ended up getting a wheelie thingo’s they use to cart around there massive speed boat engines in the pits, the hospital was about a 5 min walk from our event, so we wheeled her to ED. Although extremely embarrassing for the patient it worked quiet well and probly saved her life! -

My tosser kit was custom made... Only cause it goes from one extreme to the other. One day I could be working with an equipped BLS ambulance doing standbys the next I could be looking after 80 football players with only my kit or working an MX event with only the bare essentials, on top of all my first aid stuff I have my sports trainers stuff then a pocket for personal junk, it's like my second home basically (sad I know!) I really wouldn’t think you need to spend all that money on getting a custom bag made up if you’re only using it for EMS, there’s nothing wrong with the bags on the market, just gotta look around till you find the right one for you. I also use my bag pretty much on a daily basis. Wow I sound like a total tosser! It also weighs a tone n a half! Some of the medics and first aid guys I work with carry there gear in a kids lunch bag or a shopping bag, does the job!

HAHAHAHA that’s god damn funny! And it has the NHS logo on it, makes it even funnier! =D>

This is what they use in my service to protect all the drugs and morph, might be a bit big. But they do a great job! http://www.neann.com/AEK--drug-kit-2-web.jpg

Well my first and so far only arrest was a 68 yo male at a folk rhythm festival. He was down for about 30 seconds before we arrived. We started CPR, BVM got down an OPA and 1.5 minute in waked him with the SAED. Only took one shock and the defib told us our patient was alive. Took about 20mins for the Medics to arrive (remote area) Pt had nill hx, nill meds, looks healthy. Must have been dancing a little too hard! He managed to says “thanks” as we loaded him into the truck. I hope all my arrests are as easy as this… But yeah calls like this make you feel pretty cool lol… Good on yah Asys!
