I think my friend that using an IO for this patient is probably not warranted. Your classmates opinion of starting an IO, based on what youve posted i think is probably a little tainted by wanting to use some of those new shiny toys more than they realise, which isn't necessarily a bad thing because it shows some eagerness and it shows a willinness to get aggressive with management, and for those few times where its desperately needed thats a good thing, but more ofte than not our patients arn't that sick. The hardest thing for ambo's to do is take the minimalist approach when its an appropriate option and in this case a quiet ride, maybe stripping the pt down if they are hyperthermic and 5 minutely pats on the head with some insulin therapy at hospital is more apropriate than drilling a hole in her. Dude, in nearly 4 years of being a career paramedic i have never had the urge to say "i'm saving critical patients LIVES" Your first aid crew must be doin some hard yards man, cause after a couple of thousand jobs under my belt theres only a handful of people i had who were really critical and only 1 that i can honeslty say was saved by our direct inervention. And you do take scene safety lightly, it isn't about going in with your first aid kit, OC spray and handcuffs, its about not going there in the first place.

All the post op stuff i think would be relevent but not apples to apples methinks. No basis in my statement, just throwing it out there. Yeah, i over simplified the statement, i meant to say that people generally are not prepared to manage sepsis or septic shock until it bites them.

Meningococcal septiceamia and ICP's will give it for sepsis. Have used it as a prophylactic for open fractures (the last wound was full of horse manure) On the topic of sepsis, im amazed at how many ambo's dont recognise septic shock until it starts to bite them in the ass.

Round table duscission today, unconsicous CVA who is actively vomitng. With aised ICP and compression of the chemotactic trigger zone, would giving any 5HT3 agonists have any effect? I tried searching pubmed etc with no luck, the second you have vomiting and CVA or stroke etc in a search it throws everything at you cause vomiting is in every stroke/CVA study as a side effect and limiting the search to the title gave me nothing.

Go back to high school and finish it Enroll in a bachelors program and finish it Plan to do it properly from the start and finish it. notice a theme here?
