Went to some old duck who was complaining of feeling dizzy. I was taking her pulse and said "Well its not your heart rate is nice and regular" She says "I bloody hope so son i had a pacemaker put in last month" DUH...... I love these patient!, Its one of those job where you really get to see the effect your treatment is having! Its not very often we get to see someone go from pre-arrest to arguing about why they need to go to hospital! -

this reminds me of a story....... My first arrest was in a small rural hospital where the LMO's are GP's who work in private practice but are called into the hospital when needed. I remember standing over this guy ding compressions and next to the resus bed was an EFTPOS machine for paying the bill. I guess this pt wasn't going to be paying up before he left. My pet peeve with non emerg patients at the moment are those who call you at 3am for the tummy ache they had since yesterday, they have a government funded health care card and know they wont get a bill, so you begrudgingly take them in with their partner following behind in their perfectly usable care. My solution * Aggressive public education that ambulance do not guarantee you will be seen quicker by a Dr * Everyone will get a bill of some type, even with a health care card your gonna have to pay something, perhaps just a little more than a cab fare would cost? *Aggressive recuperation of unpaid bills *A set of defined protocol that enable us to refuse transport to ridiculous ailments - eg. sore ankle, runny nose, i think i got bitten by a spider 2 days ago and "Im farting today because i drank milk and i don't normally have milk, i think I'm having allergic reaction and i want to get checked out" (<<< did this at 3am this morning, and he wont get a bill either) Sounds like I'm a heartless little whino but the reality is that in some form there will need to be some way to cut expenditure in health care as the cost of it is rapidly becoming equal to GDP (by 2050 here they say) -

Where is this utopian service you speak of? Do they have good equipment, aggressive protocols and CE program and professional rates of pay The point is that it should be a mandated set of minimum employment conditions that are based on principles of fair and safe workplace practices, and a part of this mandate should be fatigue management of which a guaranteed rest break of 30 mins is a part of. Much like kiwi, we have a minimum set of employment conditions set out in our award, breaches of it by our employer are able to be prosecuted through the national legislated industrial authority of Fairwork Australia. A part of this is rest breaks, between shifts, maximum number of shifts in a row, and lunch breaks. We have a "window" of opportunity between 4 and 5.5 hours into our shift in which lunch should occur. If, for operational needs we work through that window, we are then placed on lunch and are only available to respond to priority 1 and 0 cases for a period of 1 hour. If your unable to get your break in during the second window, you are then are only able to respond to priority 0, the most urgent of cases until you complete an uninterrupted break. As priority 0 are fairly uncommon its very likely you will get a meal break in. in a 10 hour shift from 0800-1800 you will get your meal in at about 1430 if you over run all the windows of opportunity. I know this is a "guaranteed break" as it should be, but we are working on it. -

True, but i believe that one form of bad publicity is defensible and one is not. The way i see it is the headline is either going to be "man dies while closest ambulance crew is on lunch break" or "3 die when paramedic fall asleep at wheel, hits tree" The first headline is based on media hype and the ludicrous idea of response times as THE predictor of patient survival and reliance on it as being the KPI of choice, the other is has been and will be proven again to be negligent. Deaths related from fatigue have and continue to be found by coronial inquests here as a result of negligence on behalf of the employer not ensuring a safe working environment. -

No, i don't like it, in fact, i think your post shows the absolute hypocrisy, lack of leadership and management ideals that are so bereft of foresight and decent HR principle its should be criminally insane. Constantly, we are reminded to PSA's about driver reviving, not travelling long hours without sufficient stops to ensure you don't end up another statistic when you fall asleep at the wheel and wipe yourself and god knows who else off the planet. Truck drivers, taxi drivers, surgeons, airline pilots, bus drivers, nurses hell, even the guy at the kwik e mart is legislated on maximum number of works hours, consecutive works days, hours of between shifts and mandatory rest breaks including to opportunity to eat based on a principle of injury and liability prevention to ourselves, workmates and the public. Then there is ambulance....... For some fucking reason we are supposed to be some kind of superhuman bravado bullshit that doesn't need to rest, can go long hours without eating, using a toilet or being able to scratch you nuts. Fuck, taxi drivers are fined if they don't take adequate rest breaks but for some reason though i'm hauling sick humans and he's hauling healthy ones, im allowed to drive 14hours straight without a break and he's not, figure the sense in that. You certified genius's in management cry foul at having rest breaks because of resourcing (exceptional circumstances accepted) shortfalls, then put on more staff. Wait, my bad, you only encourage safety when it doesn't cost money Ridiculous.......... -
Answer for me is easy, which one benefits the patient the most, and if the Netherlands system or Ambulance Nurses is superior to traditional Paramedic / Ambulance Officer model of delivery, then that's what they should retain.
