Croaker, no intent to challenge your observation, I think I see what your trying to say, but many brittle severe asthmatics have a "silent chest" so would you call that asymtomatic too ? No intent to pull rank but other than working in the back of a rig have you dealt with a lot of long term care hospitalized COPD patients, your theory is a bit of a stretch although not incorrect as they all do differ, especially those on supplemental O2. The very broad term used in EMS that of "wheeze" is very often a "catch all" in fact inspiratory vs expiratory was never questioned in this nor any more PMHX nor meds. I can tell you with all sincerity that I have been informed by many Paramedics and RNs upon taking report of a wheeze, but one cough or one deep tracheal suction and suddenly the wheeze is gone, as I said prior all that wheezes is not asthma, i was intending to introduce cardiac asthma as a possible diff dx .. but when patient was texting while driving, well I suddenly lost interest. Cardiac asthma is a medical symptom, of wheezing due to congestive heart failure. cheers No cookbooker here mate, do you need respiratory distress with a history of asthma to treat a wheeze (I think your just being a pussy) LOL. With your post your suggesting MDI with spacer is less aggressive than SVN ... hmm careful there don't make me pull out my studies ! cheers

All that wheezes is not all asthma. Most incorrect, auscultation and identifying broncho-spasm is highly suggestive that patient is in status, controlled asthmatics do not wheeze. Maybe getting a bit more throughout history, like what other meds is the patient on, last ER visit, is he on any inhaled roids, when and how frequently does he need salbutamol ? Just me I would give him round of salbutamol "on the house" reason being an adrenegic response could precipitate an asthmatic event, the risk (very low) vs benefit. Just what "factors" where involved in this roll over in the first place is more of a question raised in my mind was this patient driver or just there for the roller coaster ride as an active participant? cheers

Personal or Financial .. look to "scholarly studies" for real evidence based medicine, besides just a brief overview and stating that infants are born with "sterile gut" is absolutely incorrect, meconium aspiration would not be an issue if that were true.

Well just my 2 cents, must agree with Bushy and Dwayne 100%. In an attempt to comply with this thread re: TOLERANCE again mixing the 2 cultures of Fire and Pre Hospital Emergency Medicine is a proven disaster generally speaking where I reside, divorces are far more common than long term successful marriages. Fire department have an agenda to embrace the wallet. If EMS did not generate revenue very frankly speaking then you would never see a ahem "blended" service. <insert knee slapping noises> The economics of it all, with the tightening of budgets, is that Fire looks to their own stats (the proof they use to justify their budgets) that being response to scene (the budget managers like local geopolitics are suckered in to this) and somehow the clock stops on arrival ? Whereas in EMS care the clock realistically starts when patient contact is made. The IAFF is bloody huge, the largest single labour organized entity in North America, the NFPA sets their own standards wereas in EMS its state to state/province/county/prefecture/territory and no real national solidarity, moreover subject to the local Medical Direction, scope of practice varying beyond comprehension. Now look to your own Fire/Medic stats and the massive differences in call volume(s) "medical vs real fires or public safety responses" the math just does not add up, neither does sending a hugely expensive Pumper or Crash Rescue to every "trouble breathing call" factor in that most grunt smokie's want nothing to do with that, come on its the truth "saving themselves and resources" for that 3 alarm blaze using gravity applied physics and holding a mattress firmly in situ. Unfortunately and most curiously these response times are used to justify the purchase of more Fire Trucks vs the very realistic NEED for more Ambulances, tell me that's not the truth and I will ___ fill in the blank. Bottom Line: Fire is Pubic Safety. Police is Law Enforcement. EMS is Health Care. late edit: (A tighter bond to hospital based operations for con-ed, hands on patient contact is needed in the USA, EBM studies, one cannot accomplish these goals with a "must return to the hall" the diner bell went off. Work together sure, Independent entities Yes, share infrastructure like accommodations, Yes. But having one groups priorities in funding over the other EPIC FAIL. This very serious conflict potential will undermine the goal for what is best for the people / taxpayers. That said: unless intelligent representation is made on behalf of Pre Hospital Health Care highly unlikely with the old guard in Fire Departments. The concept of innocent until proven guilty, oddly one is charged and then incarcerated until the judge decides if s/he is a threat .. if this were the idealistic .. no one would be kept behind bars.

Firstly good to see you back brentoli . So a hard and fast blanket rule sound's more like a dictated scoop and run ignoring EBM that in many cases, fast only means pronounced faster in an ER . So how much $$$ does the state kick into the ambulance operations if the government is bound and determined to save lives ... maybe drop the speed limits get more drunks off the road, bottom line is response times are not the "root cause" of fatalities on highways. I can see that this will directly result in more crashes both ground and air, I sure would like to hear Bledsoe's opinion, on the use of more choppers in the air in crappy weather ? Any mention of more funding for things like ultrasound, blood volume expanders, or more for education for Paramedics ? This pipe dream would NEVER work in Alberta, you could make the rule but unless you have a star trek tele-transporter on every rig, it just ain't reality.

Yup I know I suck with composition, quoty things and underlining sometimes, Although I am the EMT City king of the run on sentence, it is a curse maybe but I like the purple quote within a quote deal I write (so says rock_shoes) the way I speak, he is most eloquent .. but hey am even worse in person ! cheers Walk a day in a man's moccasins, before you judge him.
