  4. Ok anecdotal stories: Can I share too? I was accused of something (doest matter what it was government employee at the time) I was wrongly accused, was put off work, complied with employers every wish, finally "allowed" to return to work, after jumping through the many hoops. I filed a grievance because the accusation was bull shit and detrimental to my life and career, huge stress. The facts in the case were crystal clear that the employer did not investigate the initial complaint (an unidentified complaint most curiously) or have enough facts to even start "putting me off duty" ! Without a Union and their Lawyer I would have been screwed, blued and tattooed standing in a empty field digging ditches broke, trying to fight for my rights ... don't tell me for one milisecond that an private EMS employer will back a "worker" unless it is in the interest of the employer. So Happy I must fall into your BAD employee categorised list. In another situation I actually quit my position when a 95% union members took strike vote (I did not support I left (very simple) these in the early days of EMS in AB when EDM city took over EMS from a private operator that was under investigation for embezzlement of funds and double billing, interesting. We were working for 14 hours and were paid for 10, as the Alberta Labour Law stated at that time (its changed now oddly enough) I took a huge cut in pay left and went somewhere else as at the time I did not feel I could strike myself, sure as hell I can now but now. Then essential services clause and direct to binding arbitration so what choice does one have .. really? I went to work for a friend (who also had worked prior place) BUT he was a Private EMS Operator and during the strike HE stabbed all his friends in the back and ran one of his trucks to "make a quick buck" ... by the way he went out of business .. no union destroyed his operation his greed did. PERIOD.

FAIL in applied logic level alone. Agreed some EBM studies CPAP stays off the plastic tube and decrease mortality morbidity and lower's HC costs .. well sometimes, but implementing these plastic CPAP devices on a huge level in EMS, as if they are in any way comparable to a machine that is capable of delivering proper CPAP or BI Level support. (for that matter) Back to this new soon to be patents pending INSPIRATORY FLOW AUGMENTATION MODE .. are you interested in investors chbare? ps My Oxygen Bar and Patch making company are not doing well in this current economy cheers

I don't like to disagree, its not in my nature <insert cough> but an extreme stereotypical position on such a topic of and assuming that unions destroy companies is beyond the absurd, organisation of labour is essential to promoting any trade or profession. I don't know about your much RN affiliations/ union/ associations in the USA, but they have made huge steps in Canada and are far closer to what EMS really does, that of HEALTH care. Very curiously that "we" in EMS are of a mindset always reverting and comparing to IAFF, this is public safety as in police / LEO as if they do not have well attended unions / associations with bargaining rights. cheers

Something tells me that if I posted any number that you would not believe me anyway, rest assured I make more than your union and non union in the USA, I live and work in Alberta Canada ... like the Flag in my avatar and also in British Columbia and NWT. This would be like comparing apples to road apples. I work non-union jobs in Industry (providing services to the union and non union trades) and Union HSAA jobs in the "public" sector in the past primarily in the Air Environment. I can say emphatically and without any hesitation that the unions very positively affected the Industrial non union wage. Another point would be with the unions as in BC and major EMS services in large communities AB have a layer of job protection, it works well for a resident but against a non resident. Heck should we not have some protection in place to prevent "outsourcing" or import immigrates that have 1/2 the education and will undercut us and live 13 in a 3 bedroom home ? Any private contractor would just love to do this . Let put it this way, if you ever get up this way to visit the great white north, I will by drinks and wings.

  10. chbare: The CPAP machines are all different in means of delivery of gas, I agree with your statement entirely but it may become clearer if I continue to elaborate. Morning shift, after 8 hours drive / shovel 4 tons of snow off roof / but to late for collapsed boat garage I digress ... so bear with me zzzzzzzzzzzzzzzz LOL> A good question and its been some time since I worked with Oxylog, the flight OZ guys had some issues with sticking exhalation block on the 1000 (a failure happened to me on a LTD fixed wing) and not so popular with me any more, I hope that the 3000 is got the bugs worked out. The internal workings of the Oxylog and for some reason can not check 3000 (my Internet is in and out) .. but from memory the FiO2 controls are a combo, 1- a direct source gas limited to 50 psi FiO2 1.0 and or a .6 FiO2 (no blender) its 2- an internal venturi device and that increases flow capability (based on 50 psig tank) so on lower levels of FiO2 your actually achieving higher flow rates its highly unlikely on either setting one will have a loss of PEEP due to the patient exceeding demand. The LTV even sweeter kit, (2 to 3 times the price of an Oxylog too) it is actually a turbine support system (no matter what the FiO2 is set) the LTV senses an increase in change and literally boosts the support/ flow (can't remember exact stage flow settings there is 3) Here is the real point to this, but mobey never came back to answer the questions the EMS "CPAP" devices that are dependant on a wall mounted flow meter, (i.e compensated thorpe tube) is limited to 23 lpm hence if one is looking at the definition of a high flow system fail in the math alone and HIGHLY likely in real life as well ... 23 divided by 4 is 5.75 lpm and not a abnormally high normal MV for a spontaneous breathing adult. That said on a ventilator, the patients entire WOB is removed (the whole idea sans maintaining open airway idea) therefore decreasing metabolic demands , resulting in a lower MV overall (and at that ventilated 5.75 lpm MV) matches CO2 production and your set FiO2 is matching or exceeding oxygen demands. So in mobey speak: If you suck back hard to get a breath entirely possible one is pulling back so hardas to generate a negative intra thoracic inspratory pressure resulting in a loss of PPV / PEEP defeating the whole intent of the Constant Positive Airway Pressure idea, as it becomes an "intermittent" CPAP (only during exhalation does it provide "set" PEEP values). and in fact again one is using assesory muscle usage on "exhalation" ps entirely possible is pulling the so hardthat one has NOT decreased the WOB, assisting the major muscle of ventilation (the diaphram) Back to another point I tried to makefield level ... good clinical background and experience is the best indicator to evaluate the success OR imminent failure of any device like CPAP (although difficult to quantify in EBM studies) cheers cheers hope that helps.

An interesting development never the less my hope it does not go the way of "pretend" volume expanders that have oxygen carrying capabilities, although that stuff I suspect is a bit more "pricey" cheers

Agreed ... mostly, although a bit of a blanket last statement, there are stellar and Fire combined EMS services but those Fire services here have been infiltrated by proven Field Paramedics at upper levels, making a huge difference in priorities. If you would so allow "in addition" Response time TO patient is important but does in no way "STOP" the clock ! The public needs to know that when medical aid is to be provided the clock really just started and a means of transport is rather important in fact more essential.The FF are basing all their PR on the 100 yard dash, vs the Paramedics are the little known "800" not a dash nor a marathon runner in Patient care. cheers

Agreed Ruff: Where I pay taxes we have a 3 million dollar Firehall (plus 4 trucks) the call volume 30 calls a year serving a population of 10,000 folks, highest level of training is EMR (very basic first aider) and only 2 vollies FF out of 38 have that advanced level of care. Closest "2" ALS unit's are a 20 minute response time away best case scenario and serving a population of < 100,0000. Do the math in my hood and it appears something is wrong .
