The most optimal uptake via neb for most broncho dilators is ~ 10 to 15 % efficacy. I have nebulized Morphine (bronchogenic carcinoma) but I have never heard of narcan neb. Why one would deliver narcan this way is beyond me when one can deliver narcan IV/IM/SC/ET. Maybe something I have never heard of ??? cheers

I think your taking that general question a bit to literally, following your post and AK logic should we then assume that "privatizing EMS" is hiding shit and dastardly deeds ? I can say without too much reservation that I presently prefer to work for private contractors than what we have in my hood right now, hey wait a second the vast majority of services here remain sub contracted but hidden under the smoke and mirrors of a provincial government and forced takeover of EMS, with a forced union of the governments choice a most very interesting situation, ain't it lots of eyebrows raised over that issue. It is appearing (who really knows) that the my "state government" has perhaps not done their financial homework and far over budget go figure EH ? Although they have managed somehow to introduce yet of another layer of middle managers whose job is to eventually eliminate their very own positions over the long run, this under the auspice of a Health Care Centralization concept, once again most curious. I must ponder just how hard the white shirts are working on that then again perhaps this is a means to an end and eventually privatize the works like other provinces claiming its too expensive and should be farmed out ? ps I am from the Government and I am here to help. That said an election is looming im my hood, we just may see a very effective opposition and soon with very difficult questions to be answered in the hallowed halls of granite and concrete, I can only hope for democratic process. EMS in my hood has always been the Bastard Son of Health Care and historically speaking the so called PRIVATES here have promoted advanced life support care and FAR FAR more than our present "self acclaimed global leaders". I sure wish they (the government) would fund that bunch instead of legislating me to pay to accomplish their agenda, it sure isn't mine. cheers -

Although I think I agree with most of this post on a conceptually basis, if one is pro privatization why would one send out a warning for government run entities ? This generates some questions in my mind and knowing that private vs public delivery of any "Health Care System" this polerizes many ideologically from the get go. For example in Canada any hint of this immediately raises eyebrows although we have ALWAYS have had a blended system. This topic is dear to the hearts and minds and akin to the USA and the Right to Bear Arms, nuff said. One has to ask should EMS be considered a business or an essential service ? Does a country "privatize" their Armed Forces, Coast Guard or National Guard ? IMHO believe that Pre Hospital Care should become more aligned with Health Care no matter how it is funded and away from Public Safety Services. Privatizing the Fire Department's could be a very cost effective concept, although it will be met with huge opposition by the organised crime er IAFF. I have no reservations asking for comments or different perspectives. -

AHEM ... The R.R.T.s prefer the terminology "Therapists" as this is not a "fix the machine" career, it is applied sciences with technology TO the patient <end pet peeve rant> Oh so off topic but in the long run the terms used to legally identify the "practitioner" in many health profession's ... well just look on any medical career website one will find that biggest difference between Technologist to Therapist is the PAY SCALE plus national / international recognition, oh yeah did I mention PAY SCALE ? I find is very curious that when the topic of what type of "tubes" one should purchase that LUNG SOUNDS take a back seat ? Lung sounds which in many cases ie do I dare say diagnosing a "Tension Pneumo" recognition is far more significant and even though I dislike the term Life Saving, it defiantly does apply in this pathology. In vast majority of cases the only exception (I can think of at this point) is identifying "muffled heart sounds + patient presentation + PMHX) That said: IF one has protocols that permit pericardiocentesis (and loosing favour, btw) and WAY beyond the EMT student level) Most seriously a Paramedic or EMT can do jack **** about a murmur or gallop, although understanding the problems associated with abnormal heart sounds can be helpful and very interesting. SO hit the books and best do your homework or AK will bust your hump OUCH. Agreed: For example when evaluating a murmur's the "quieter" the murmur the bigger the leak ... just saying as if in EMS we can do anything about incompetent valves in the first place, and attempting to report what grade of murmur does raise some eyebrows. I have a selection of tubes but always rely the II classic. Why ? .... Because it fits in my back pocket and I can sit on it without too much discomfort, cardio can most times just be a nuisance and another point ... if I loose a set, I not going to cry AS much. Thing is: Shiney expensive new tubes have a tendency to sprout legs and walk away. I etch my name on the inside of the (diaphragm side) kinda sneaky and under the diaphragm hidden till one is sorting stuff out from messy call, as those plastic name tags are easy to remove. That said: I know of a shiny new Medic that has a $600.00 set of recording cardio tubes hanging around his neck at all times, well "SUPER DAVE" as we call him, is proof positive that the more you spend does not equate to ability or knowledge or experience. cheers

Lets just imagine for a minute that the funds that are donated to this cause, that those funds could be used to a far more noble cause ? I post this while watching SU2C thats: Stand Up To Cancer. Yet instead to prevent Lawyers bulling our site that is dedicated to sharing of EMS information and directly pertaining to our culture. Just look at the same situation for example in cancer, cardiac, CPR, stroke and a host of other illnesses and the research that is done and openly shared to prevent death and suffering. We that are health care professionals simply can not fathom that this selfish act by a news paper conglomerate. For SHAME Wronghaven you truly SUCK ! -
Ever see a King or Combi used in ICU ? Ever see a #4.0 King Tube ? It appears that once again plastic technology wins over education, practice and proven experience ... and then the second procedure exposing the patient to increased possibility of aspiration. This debate is noting to do with "old school" or "egos" it has do do with what is the best and safest procedures, but it does have a LOT to do with marketing and studies based on less than stellar services. cheers

Words from Kevkie a respected member of EMT City, but worth modifying (a titch) and re posting, you would never find this advice in a search. One could print that out and hand it to your "new partner" at the end of your next shift, I doubt it would hurt. Perhaps its too late in the process as their sounds to be a bit of a trust issue ? I would suggest that you ask the "trainee" to constructively critic himself and use "good listening skills" yourself. That said: If the trainee is ignoring your advice you are the FTO and this ain't farking high school its your responsibility to maintain high standards, even though some times one does have draw a line. Oh shucks where are my manners WELCOME to EMT City ... Is an apology is in order? Perhaps a tad bit too sarcastic and premature response from myself. Most sincerely I really don't wish to rain on anyones parade or dream(s). I have pondered about this first post a bit so I have to ask where are you located and is it possible that English is not your first language as I have friends in the Caribbean island's that would ask just this type of question. I am still astounded how well they run their volunteer services on little more than the change in their pockets for petrol. In todays economic age to start a service in North America is not as simple as stocking an ambulance, there are many of services that are finding it very difficult to keep afloat. Start up Capital, then a Business Plan for Banking Credit (unless one is independently wealthy) Insurance, Regulations pertaining to the state/ province/ territory or parish. Staffing, Training , QA/QC, and finding an MD willing to back you all come before ordering "kit" on-line. So a question, are you located on the North American Continent gtl if you are not please PM me and I would be pleased to provide numerous equipment list(s) that I have developed for off shore use and how too make spine-boards and make cravat's out of bulk cloth . cheers

I too can remember that day, in my day it was not formal classes it was get in the "Bonnie" we have a "TRIP" .... and its been quite a ride. I would say this is exactly what the civil rights lawyers are attempting to suggest, got to love the "BIGOT" card played whenever the remotest trace of a perceived violation of a rights issue. Taking that just one baby step further is that the polls are saying than New Yorkers (70 % of all denominations) are Bigots then ? Agreed you have done your homework their rock slippers. The BC jurisprudence exam is 25 questions (on-line) after the proper ID and confirmation of "registry" or licencing from another province recognized under the AIT agreement. Getting the Licence is a cake walk coming from AB ... going the other way and having to deal with the AB College ... I can only imagine. ps and its just 100 bucks ! Getting hired in BC other than industrial is somewhat more complex seeing as the BCAS has exclusivity (presently ) and the postings for ACP are no where near as numerous in opportunities ... then there is the union stuff in hiring, being seniority in the hierarchy (of which there is no cross border recognition) Oddly enough the CCP level (according to BC licencing) is not up for reciprocity which is quite intriguing .. I personally enjoy a paper challenge. cheers

Unfortunately the government is now involved from the neighborhood crime watch to your President, and this proposed building has hit dang every mainstream news media in the world. Even Obama has now modified his juxtaposition on the WISDOM of this proposed building and its location. BUT At the end of the day in this discussion one must ask themselves just one question: Have the "moderate" Muslims accomplished what they have supposedly set out to do ?
