Dear Reverend: So change the "scenario" just a bit, as in there is no Trauma Center to Load and Scoot TOO. Well accept for an intermediate level of care and basic clinic with very outdated diagnostic equipment and lucky if you can find an xray tech, this this within (3 hours by ground) as Helos do not fly here at night (well yet) and then an additional 2-3 hours by Air (fixed wing)to the almighty Trauma Center. So are we educated enough or have enough of the right type of equipment for an accurate dx or give treatment that will not make blood into kool aid, should we stick to our Load and Go philosophy or look for ways to improve care, just saying when faced with this situation one is forced to look to other philosophic means to solve the problem. This is a very realistic situation that I find myself in and daily when I am remotely deployed and then I hear the "Golden Hour" quoted by the clowns in safety and Medical Directors (that are located a thousand miles away) those that dictate and limit my actions and treatment options. Any positive Ideas as to how to improve chances of survival for my patients, besides tranexamic acid, volume expanders, or Hypertonic Saline + . Just trying to think outside the "Golden Hour" or "Load and Go" box. I wish to introduce the term "Canadian Silver Six" cheers

I certainly don't see this group going after any major companies that have equal legal power. I have 2 friends that "past tense" were Paramedics and then became DDS (a far better paycheck btw) they also do conscious sedation (I bet both push more meds and start IVs than most in EMS do in a day) and both are ACLS instructors, both would be a good choice in a major wreck. One even has his commercial aviation licence and his own twin engined pressurized aircraft that would be an added bonus .... most certainly. Dentists are a major supporters and economic supporter of CSPS (the Canadian Ski Patrol System) and have been supporters of the Blood Samples Act, so that we ALL in Canada can gain access to information on a "bug eater patient" that refuses or is unconscious, to provide a sample of blood, in the event that any health care provider inadvertently gets a needle stick. I am not certain that its nitric as much as citrates used as a preservative's hence a metabolic acidosis (but looking at the books AGAIN) ... plus it gives me one headache doing dialysis and checking ionized calcuim every dang sample. Citrates screws up clotting factors rest assured, but how else can one stop RBCs in the bag (from becoming a blood pudding) but we can overcome or at least compensate with platelets and the other goodies ... available in Hospital only (at this time) At least the brits are giving a good run with tranexamic acid, who knows maybe we can stop rushing to hospital and saving our own lives with the positive diesel sign ? cheers cheers
