  1. Just confirm, Ben. You have some autonomy for your actions as what I would call Paramedic Scope of Practice, without needing to call in for On Line Medical Control, i e "Mother/Father, May I?" Due to distances, and possibility to need Helicopter EMS a bunch more than I would, due to my being "urban/metropolis (/Megalopolis?)" versus your "country/agricultural (/wilderness?)", it would make sense that Paramedics would, and should, be higher trained.
  2. The following E-Mail was sent, with my full name and address included, to Congressman Robert Turner, of the 9th Congressional District, formerly held by the disgraced Anthony Weiner. I believe the letter is self explanitory.
  3. Almost? As son of a teacher (Momma B, NYC Department of Education), brother (sister retired NYC DoE, brother a History Professor at University of North Dakota), grandmother (Assistant Principal NYC Board of Education), and boyfriend of another teacher (Lady J), figure it to be 99.99999 per cent the case.
  4. Lady J is a teacher. Gotta show her this, because, even if it is a parody of real commercials, it speaks TRUTH!
  5. Lady J.s mom is a double leg amputee. That happened after her husband passed away. As both Lady J, and her brother were away from the house most of the day, working, and she felt too afraid to use her prosthetics, and either couldn't or wouldn't leave the house, she surprised her kids, Momma B, and I, by deciding to go into a local extended care facility/rehab center. Within weeks, she became either a member of every organization in the place, or elected to a position within the organization. Sounds quite dissimilar to a cucumber, huh, folks?
  6. I had a situation where a hand signal could have come in handy. My partner was outside the ambulance talking to the LEOs, at the scene of a man who'd been assaulted. I had the man inside the ambulance, alone, checking him out while he was seated on the "crew bench", when I noted the man was carrying a firearm! How do you advise partners and the cops that you need them, without alerting the patient that you know they're armed? I was lucky, as the patient asked for me to summon one of the uniformed LEOs, as it turned out he was an undercover, plain-clothes detective, from a different precinct than where the call was.He surrendered his weapon to the LEO Sargent on the scene.
  7. When my dad died, I have no idea how long the ambulance and ER crew had been working him up, prior to my Captain driving me to the ER to meet up with my mom. Resuscitative efforts had already been terminated. I was talking about organ donation with her, when the on-duty transplant coordinating nurse walked up to us, and joined our conversation. While my dad was a diabetic cardiac, the "harvesting" team that later worked my dad's remains, at the Queens County Medical Examiner's office, got skin for burn victims, and 4 people got partial corneal transplants. I don't recall if they tried for the kidneys.
  8. You don't want me directly quoting agency, local, or state protocols, so I'll paraphrase a bit. No CPR if obvious death, like a decapitation. Judgment call if Dependant lividity seen. Had one where the extended care facility called about a patient having difficulty breathing, described as "breathing noisily", but they were about to call 9-1-1 back as the patient seemed now to be quiet. He was quiet, and also displaying lividity. I followed protocols, determined for "Presumption of Death" per department, local, and state protocols, then asked my partner to confirm by doing the same. When he did, I said to him, "confirmed for DOA at...Hours", only to hear the gasp from the facility charge nurse behind me. She had observed the noisy breathing, and wanted us to check the patient out, when he apparently was having his "death rattles". The call to 9-1-1 was made almost an hour after the observation, and my ambulance was at the intersection just across the street from the facility, so it was directly from "Dispatch" to "On Scene", no "On the way" signal transmitted.

In all my years, I thought the signal for "all crews to evacuate immediate scene" was all drivers vehicle operators/chauffeurs to "tie down" the air horns and sirens on their (all agencies) apparatus for about a minute. That would be a holdover from before everyone had a portable radio, and possibly still used as a backup if there is a radio "dead zone". 3 Air horn blasts is a new one on me, but it could be a local protocol.

  11. Just mentioning that, with my Lady J, she caught on early that I not only implied the offer of exclusivity, but also asked it. However, we were in our 30s, with no potential other guy or gal, and no heavy baggage such as the OP mentions. Different frame of mind. (Cue the music: Two less lonely people in the world) A possible problem is, if a demand is indicated, of "Him or me", you might lose her. Different problem is, she stays with you, HE might come dangerously after you. Keep pointing out the "bad seed's" potential problems to her, and hope that she is mature enough to realize the potential error of her ways. Hard as it may be for you, depending on your feelings, you might have to close the door on her, and yes, I know how hard that might be.
  12. Boondock saints? That the movie about the Irish twins in, I think, Boston, go around shooting "evil folks" that the courts had declared "Not Guilty"? As I said, the Blanket Party I mentioned was told me by the former girlfriend, but out of protecting both her and the other former boyfriend, I have to question her veracity. She had told a bunch of great tall tales when we had been dating, but many of them actually were true. I have no idea regarding that story if true or not, although the Statute of Limitations may have run out by now.
  13. I'll notify Detective Olivia Benson from NYPD Manhattan Special Victims Unit. Seriously, if the girlfriend has a thing for bad boys such as you describe, best read her the "Riot Act". Worst case scenario, which I mention but never recommend, throw the guy a "Blanket Party" (Have a group of people acting in concert approach from behind, throw a blanket over his head, and pummel him till he drops, the blanket so the "guest of honor: doesn't know who assailed them). Rumor mill has it that the now former husband of a girl I used to date had one thrown for him by another former boyfriend, after the husband wronged her, and she divorced the SOB. Before she married, everyone had thought she was going to marry the boyfriend. The girl mentioned the incident to me several years after the act allegedly happened. edited for accuracy-
  14. I'm observing from a possibly dated BLS viewpoint, via NY State and FDNY protocols. Sudden Cardiac Arrests, after the ALS does their "thang", if no ROSC, they contact OLMC for permission to terminate resuscitation.

  16. Please note that you're in NSW. We have established, in other strings, that outside of the US, BLS includes a bunch of good stuff that, if I were to do here in New York City, I'd get nailed to the wall for being outside of "Scope of Practice". Some examples are, as an EMT-Basic, I am not allowed to start an IV. I'm only allowed (under state protocols) to administer albuterol, 81 Mg chewable aspirins, glucose paste, and oxygen. BLS crews are not YET allowed to do finger sticks, as ALS only recently got the OK to do so, but my instructors foresee it coming. Going back to IVs, we BLS providers in the state of New York used to be allowed to transport patients on an IV, as we got training on maintaining the flow, and orders to stop the IV (with notation of the time) if we found the line to be infiltrating. A few years ago, that was taken from us in favor of a placeholder IV started. I'm blanking on the name at the moment, but it is just an IV needle filled with saline, if memory serves.
  17. Seriously, now. Is the person dressed appropriately for local weather conditions, weather as appearing as a rich man or homeless? Is the person exhibiting odors of excrement, urine, or alcoholic-like substances? Good, someone gloved up, and attempted to ascertain level of consciousness. What was the response? Did the driver see anything, or just call in a "man down, may need assistance" type request for aid? Did anyone, as a bystander, see anything, and if so, what? If in local protocols, and in level of practice, are you going to do a blood sugar level finger stick test?

  19. I just realized something: I said the Murray "Yellow book". Should have indicated "Brady" published what "Murray" wrote, so possibly I may use either name interchangeably. Also, Dr. Caroline's book is ALS. While I, as a BLS provider, have seen the book, it was always in possession of an EMT-P. I never had chance to open it and read.

As I recall, from Midwinter to Spring, 1974, when using the two "First Edition" texts, the AAOS "Orange Book" (Emergency Care and Transportation of the Sick and Injured) was written as a fairly dry college textbook, and the Murray "Yellow Book" (Emergency Care) was more of an illustrated comic book. We must agree to disagree on this particular topic.
