well here it is at 4:30 am this morning i was called to a local nursing home (i wont name, names) and the complaint is that a pt had received the wrong meds. on arrival. the patient is waiting for us outside. we asked what was going on and he said" how the hell should i know, they (the nurses) just woke me up." so i ask the nurse what the deal is. she says he was given 1mg of atavane instead of his zanex. i asked what time. she said 10 30.

Zanex? or Xanax?

10 30 AM? or 2230 PM?

Spelling and timing are two big causes of medication errors.

I'd imagine this is a common threadamong every EMS, but i've seen about 15 variations on Fentanyl come out of my dispatchers. Antibiotics and Antineoplastics are even better...
