The most common form of writing one's pregnancy Hx in the hospitals is as follows:

Gravida ( # of pregnancies)

Term

Preterm

Abortions (this will include any spontaneous abortions (miscarriage), ectopic pregnancies (tubal pregnancy)

Living

For ex: A mother comes in and says that she had an ectopic pregnancy, she had twins @ 27wks, and one full term. Her pregnancy hx would be listed as: G 3 T 1 P 1 A 1 L 3

Yeah, learned that the hard way. I asked a 14 yo if there was any chance she was pregnant on a syncope call in front of her dad. He went ballistic on scene and in the ER when they asked the same thing.

Turns out Dad was the Dad. :shock:

You may want to ask about any recent intercourse. For some reason that tends to be forgotten. And even object insertion.

Ask of any change in diet, especially green leafy veggies.

Those are the one's that stick out first in my head when I first saw the topic.

to interject some levity in this thread, you can always ask "Are you sure you are pregnant?"
