Actually there are several claims as to its origin. It was a discussion on this site some time back.

Take a Ford Econoline 150 van, not new. Put a light bar on it. Mount a stretcher in the back. Stuff in the minimal equipment required for a license, mostly the portable versions of whatever you can find. Put some supplies in a duffle bag. Mount an old bass boat seat for the nurse sit. Use the largest star of life decals you can find. And boom, there's your new transfer unit.

And I'm not exaggerating.
