Haven't they heard of a tail gate net? If the board is too long to close the tail gate, they should have had straps/nets up. That's what we did. Yes, guilty. Actually we were on an oil rig about 300 yards from the road. It was either a 4x4 truck or the bull dozer. Believe it or not, no one there knew how to drive the "cat", but me, and I had to stay with the patient.

By the way, I grew up in the oil field. Nuff said.

1) Failure for no long board patient restraint to hold it in the vehicle.

2) Failure for the attendant not being restrained, and falling out of the truck as well.

3) Failure for the driver, for the jackrabbit start, resulting in the unrestrained back board patient and unrestrained attendant falling out of the truck.

4) Failure to MYSELF, for laughing so hard at the misfortune of the back boarded patient and attendant falling out of the truck, with no regard for whatever injuries they received in the fall.
