Kiwiology Posted June 11, 2012 Posted June 11, 2012 For patients with tachydysrhythmia who are significantly compromised the treatment of choice should be cardioversion particularly if the rhythm is thought to VT "Significant" is subjective but requires more than "a bit of chest tightness or SOB" - basically think patients who are cardiogenically shocked or going to end up that way, so the small, elderly or other patients with limited physiological reserve Now sedation is a bit of a funny issue, in the hospital here the choice is midaz or propofol. In the pre hospital environment here we have midaz, fentanyl and ketamine. My personal preference is for IV midazolam and top it off with a bit of ketamine Paramedic Officers can give some IV or IN fentanyl then cardiovert now, for years (like nearly twenty years) they could cardiovert but not give analgesia but my take on that is if you are that bloody crook you need cardioverthing you wont mind a bit of pain for some extra years of life I do not think there is a role for prehospital blockers be they calcium, beta, sodium or otherwise; there may be a small role for beta blockers for MI patients but I am not familiar with the literature suggesting improved survival but I do believe some exists

Kiwiology Posted June 14, 2012 Posted June 14, 2012 That is far out choice bro no ghost chups required now can offer direct Traverse City, tango victor charlie, then direct Waterloo um, yankee wiskey tango, call Toronto Centre one two five point eight seven, so long I am anal I don't use the navaid names I generally always use the identifiers especially from working internationally in places like Japan you will get names you do not know how to pronounce infact when I was reviewing documentation for Northwest (well, Delta now) it said to not attempt pronunciation and use the identifier. Once you get east of Poland and start needing to use a feet to metres conversion chart things get a little hairy ....
