Perhaps a physician views treatment as an overall assessment and decision, whereas an EMT views it as more of an algorythm, so a physican would be more willing to classify it was "not the right move" but not an error while the EMT messing up usually means he failed to follow a specific protocol, so it's an error? At least in part.
