You are snug in your bunk after 16 hours of sitting on your butt watching "General Hospital" reruns. You have not had such a perfect shift, and you start to doze off, and in a dream you finally realize the Sarah is alive and her twin sister seperated at birth is actually the "Phantom Killer." :lol: Then, your perfect day is ruined by that evil tone. Your unit is called to the scene of a 64 year old male having chest pain. What would you like to know?

Anything out of the ordinary for the neighborhood?

Where is the patient found? What does he look like?

Responsiveness? Respiratory effort? Skin color/condition?

Quiet night in the neighborhood, nothing out of place.

Patient is found sitting up in bed.

Patient is AO times 4, he looks pale, and complains of chest pain and tightness with dyspnea. His ariway is patient and he is breathing at about 24 times a minute.

What time did it start? (what time is it now?)

What was he doing at time of onset - watching General Hospital? mowing grass? cuddling with the wife? the Boogie Man jumped out of the woodwork??

Cardiac history (or other significant history)?

Location/radiation/quality of the pain?

Does he have NTG prescribed? If so, has he taken any?

Medication list (any sildenafil citrate?)

History of trauma?

Vitals?

-The pain started suddenly at 21:30 it is now 22:00.

-He was sitting down watching the television.

-He states he had a heart attack 6 days ago and was discharged from the hospital yesterday.

-The pain is located over his heart and radiates into the substernal area. He states it is hard to describe it, but "it feels very different from his heart attack and he just does not feel good at all."

-He does have NTG tablets but misplaced them.

-He misplaced his meds as well, but states he takes a blood pressure/heart pill and an aspirin every day.

-No history of trauma noted.

-Pulse: 110 weak slightly irreg at the radial. RR: 22-24 moderately labored. B/P: 88/46. SAO2: 90% on room air. Pain: 9/10. Skin: pale and starting to look diaphoretic.

JVD? Hooding? Tracheal deviation?

Oxygen NRB/IVs/ECG--12 lead if possible/BGL

Move quickly to transporting unit.
